Leslie is the owner and Administrator of Community Choice Home Care, Inc.
She established the agency in 1996 and has been a nurse since 1991.
Thea Floyd, RN, MSN, Clinical Nurse
Thea has been a nurse at Community Choice Home Care, Inc. since 1996. She is an ACLS Provider as well as a CPR and First Aide Instructor. Thea completed her nursing degrees at Scioto County Joint Vocational School for LPN and Hocking College for Associate degree in nursing. She has completed her Master's Degree in Nursing with Licensure as a nurse practitioner.
Josh Smith, Assistant Administrator, Information Technology Manager
Josh has been with Community Choice Home Care, Inc. since 2000 as Payroll and Billing Coordinator. Josh is Microsoft™ Certified since 2011.
Melissa Smith, RN, BSN, MHA clinical Director
Missy Smith has been an employee with Community Choice Home Care, Inc. since 2001. She began working as a home health aide while attending nursing school and after obtaining her Associate degree she began working full time as a registered nurse. Missy received her BSN in 2009 and her Master's in Health Administration in 2012.
Alisha Barnard, RN, bsn, Scheduling Coordinator
Alisha has been a registered nurse since 2006 and has been employed with Community Choice Home Care, Inc. since 2006. Alisha completed her associate degree in nursing at the University of Rio Grande and completed Bachelor's of Science in Nursing with Ohio University.
Shannon Begley, LPN, On Call Nurse
Shannon has been employed with Community Choice Home Care, Inc. since March 2012 has completed an Associate Degree in Nursing at Shawnee State University. In addition to her nursing degree she has an Associate Degree in Applied Business from Southeastern Business College.
Katherine Cunningham, Quality assurance clerk
Katherine has been employed with Community Choice Home Care Since April 2011. She received her Bachelor's Degree in Psychology and a minor in Social Work from Shawnee State University and is attending graduate school online at Simmon's University pursuing degree for her Master's in Social Work.
TINA dILLON, RRT, Office manager
Tina has been employed with Community Choice
since March 2017. She is a 12 year Air Force Veteran and obtained her Associate Degree in Respiratory Therapy
from Shawnee State University in 2005.
